The patient has an enlarged heart.
The important abnormality to observe is the small cavitating mass at the left lung apex.
If you look carefully you can see that this has eroded the underlying posterior aspect of the left 3rd rib...suggesting an agressive (malignant) lesion.
This was subsequently biopsied under CT guidance and shown to be a primary squamous carcinoma.
