The fluid and air filled structure arising from the right larynx is a laryngocele. These arise when the saccule of the laryngeal vaentricle becomes functionally obstructed (e.g. in players of a wind-instrument), or through obstruction at the opening of the ventricle. This is often due to a tumour at the laryngeal ventricle and clinicians should be directed to inspect this region carefully. In this case the laryngocele was due to a small laryngeal ventricle tumour which is difficult to appreciate on the scan due to the distortion of the anatomy.
